            ACKNOWLEDGEMENT OF RULES/TRANSPORTATION CONSENT

               

This form must be signed by both the student and the parent/guardian and be on file at Floresville High School before the student may participate in any University Interscholastic League (UIL) or Texas Math and Science Association training session, practice meet, or contest.

Parent/Guardian’s Permit

 

                I hereby give my consent for the student named above to compete in UIL and Texas Math and Science Coaches Association academic contests and to travel with the coach or other representative of the school on any trips.  Neither the UIL, the Texas Math and Science Coaches Association, nor Floresville ISD assumes any responsibility in case an accident occurs. 

                Students representing FHS while on these trips will comply with all rules and regulations set forth by FISD as stated in the student handbook.

                According to UIL standards, students are eligible to represent their school in interscholastic activities if the student:

* is not a high school graduate,

* is a full-time, day student in the participant school the student represents,

* has been in regular attendance at the participant school since the sixth day of class of the present school year or has been enrolled and in regular attendance for 15 or more calendar days before the contest or competition,

* is in compliance with state law regarding grades and credit requirements, and rules of the State Board of Education,

* has the required number of credits for eligibility during the first six weeks of school,

* is enrolled in a four year, normal program of high school courses,

* initially enrolled in the ninth grade not more than four years ago, nor in the tenth grade more than three years ago,

* was not recruited,

* and did not change schools for the purpose of participating in a UIL academic contest.

                Students providing their own transportation to and/or from any academic seminar, practice, or contest must provide a written statement from a parent/guardian authorizing parental permission and have an approved alternate transportation permission slip.  Riding in a vehicle with another student will require written permission from the parent/guardian of both students.

                If a student declines to participate after the student’s authorization to remit fees for participation in an academic seminar or meet and the school has remitted said fee on the student’s behalf, the student will be deemed responsible for reimbursing the school the said fee.

                If, in the judgment of any representative of the school, the student named above needs immediate care and treatment as a result of any injury or sickness, I do herby request, authorize, and consent to such care and treatment as may be given to said student by any physician, nurse, hospital, or school representative; and I do hereby agree to indemnify and save harmless the school and any school representative from any claim by any person whosoever on account of such care and treatment of said student.

               

                I have read and understand the rules and guidelines state above and agree that my son/daughter will abide by all rules and regulations set forth by Floresville High School, UIL, and Texas Math and Science Coaches Association.
